MYF Membership Commitment

I hereby affirm that, in accordance with the Constitution and Bye-Laws of the Methodist Youth Fellowship (MYF), if I fail to attend meetings, pay my dues, or participate in any MYF activities for a continuous period of one quarter (3 Months) without a valid reason or without the knowledge of any executive member or fellow member, I shall consider myself as having forfeited my membership in the MYF.
